CHN PPO is a PPO, which is a network of providers and hospitals. CHN PPO is not an insurance company or claims administrator, therefore we do not pay claims.

The CHN PPO Network will be utilized by many companies. Each company has their own set of benefits. You should either refer to your benefit book, your policy of insurance, plan documents or contact your benefits administrator to verify your benefits.

Please remember: Although a provider may be represented in the CHN PPO Directory, it does not necessarily mean that services they render or the service area in which they are located will be covered by your plan or policy. Please be sure to contact your Benefits Administrator or Personnel Office, or Carrier.

Notice: The providers listed on this roster are independent contractors and CHN PPO is not responsible for healthcare provided or the omission of the provision of healthcare by these providers. CHN PPO does not in any manner interfere with or participate in the provider/patient relationship and all healthcare decisions are between the patient and provider. The selection of a provider is the obligation and decision of the patient and not based upon the credentialing or any recommendation by CHN PPO.

There are hospitals that have agreements with CHN PPO that do not include physician services for x-rays, laboratory testing, anesthesia, and/or emergency room care. There may be other healthcare professionals, such as physical therapists and occupational therapists, that work in the hospital but are not employees of the hospital and charge separately for their services and choose not to have an agreement with CHN PPO.
